Dr. Zeh will have a free consultation with you before he does an exam or any treatments that he deems necessary.

Your First Appointment

Your first appointment will take about 45 minutes to an hour. Filing out our Intake Form before your visit will save you about 10-15 minutes off of that time. If you don’t have time to fill it out beforehand, you can do it here in the office.

Casual clothing is best. We have gowns for x-rays in case you need to remove any clothing or jewelry.

Please bring your ID and any insurance cards to your first visit.

We offer our paperwork online so you can complete it in the convenience of your own home. Please complete our Intake Form prior to your first visit, this will save you 15-20 minutes.
